Bullying and Harassment Report Form (Blue Form)

Please use the following form to report acts of bullying, harassment, or other discrimination.  These can be submitted anonymously or with specific contact information.  Paper forms can be obtained in the front office.

Please keep students and staff safe by reporting any acts or threats of violence.  These may be reported to the SafeOregon tipline:

SafeOregon Tip Line

SAFEOREGON is a private tip line that is checked 24/7.  Your name will not be shared unless you want to share it. People who work at SafeOregon will look at your tip and send it to school officials and possibly law enforcement. 

You can use SafeOregon to report: • Bullying in person • Bullying online • Safety threats • Fighting • Drugs • Alcohol • Weapons • Talk of suicide, depression, or people who want to hurt themselves or others • Abuse • Sexual assault

Tips can be reported four ways:
